Certification of Information – I understand that I am applying for employment with Tech Nation. By signing below, I declare that the information I provide in the application process is complete, true and correct to the best of my knowledge. I understand and agree that any false information, omission or misrepresentation, whether discovered before or after a job offer, can result in rejection of my application or termination of my employment.

Purpose of Application - I understand that this application form is for use in evaluating my qualifications for employment; it is not an offer or a promise of employment. A background investigation, interview, various tests and reviews may be required before any final determination of my suitability for employment is made. All employment at Tech Nation is “at will” and is not for any fixed time. If employed, Tech Nation may terminate my employment at any time, with or without cause. I understand that approval of my application means only that I am eligible to be employed based on the needs and job openings of Tech Nation customers. When suitable work becomes available, Tech Nation or its agents will notify me.

Background Authorization – The information contained in this application is correct to the best of my knowledge. I hereby authorize Tech Nation and its designated agents and representatives to conduct a comprehensive review of my background for employment purposes. I understand that the scope of the background report may include, but is not limited to the following areas: verification of social security number; current and previous residences; employment history, education background, character references; drug testing, civil and criminal history records from any criminal justice agency in any or all federal, state, county jurisdictions; driving records, birth records, and any other public records.

I further authorize any individual, company, firm, corporation, or public agency to divulge any and all information, verbal or written, pertaining to me, to Summit Skilled Trades or its agents.

I further authorize the complete release of any records or data pertaining to me which the individual, company, firm, corporation, or public agency may have, to include information or data received from other sources. Tech Nation and its designated agents and representatives shall maintain all information received from this authorization in a confidential manner to protect the applicant’s personal information, including, but not limited to, addresses, social security numbers, and dates of birth.

Consent and Release for Drug and Alcohol Testing:

• I, Applicant, understand that pursuant to Tech Nation Policy for a Drug and Alcohol-Free Workplace, I am being required to submit to a drug-screening test.
• I hereby consent to submit to urinalysis, breath, blood, and/or Tech Nation shall determine other tests as for the purpose of determining the use of illegal drugs or alcohol.
• I understand that it is the current illegal use of drugs and/or abuse of alcohol that prohibits me from obtaining employment with the Tech Nation.
• I agree to hold Tech Nation and its agents harmless from any liability arising in whole or part out of the collection of specimens, testing, and use of the information from said testing for consideration of my continuing employment.
• I agree that a reproduced copy of this consent and release form shall have the same force and effect as the original.
• I have carefully read the foregoing and fully understand its contents. I acknowledge that my signing of this consent and release form is a voluntary act on my part and that I have not been coerced into signing this document by anyone.
• I understand that Tech Nation enforces a Drug & Alcohol Policy, which prohibits the possession, sale, use or being under the influence of alcohol, or drugs during company time,other than the use of prescribed drugs. Violation of this policy will lead to my immediate dismissal.
• I understand that failure or refusal to cooperate with any of the above-prescribed procedures for any reason shall constitute serious misconduct of the policies of Tech Nation and I will be subject to immediate termination of employment.
• I understand that submission to a drug/alcohol test in accordance with established policy is a condition of employment with Tech Nation and consent to provide a urine and/or blood specimen for drug and/or alcohol testing as provided above when requested by Tech Nation.

Assigned Employee Confidentiality Agreement:

• I will not use, disclose, or in any way reveal or disseminate to unauthorized parties any information I gain through contact with materials or documents that are made available through my assignment or which I learn about during such assignment.
• I will not disclose or in any way reveal or disseminate any information pertaining to the company I am assigned or its operating methods and procedures that come to my attention as a result of this assignment.
• Under no circumstances will I remove physical or electronic documents or copies of documents from the premises of the company I am assigned.
• I understand that I will be responsible for any direct or consequential damages resulting from any violation of this Agreement.
• The obligations of this Agreement will survive my employment by Tech Nation.

I hereby authorize you and all former employers, and others given by me as a reference, to answer all questions and to give all information in connection with this application or in any way concerning me. I agree, if employed by you, that if ever I make claims against you for personal injuries, upon your request I shall submit to drug screens and examinations by physicians of your selection. Your employment of me may be terminated by you at any time without any liability to me except for wages and salary as have been earned by me at the date of such termination. I understand that it is my responsibility to notify you of my availability on a weekly basis at a minimum, and if I do not, I will be considered unavailable for work.
